Top 15 IVF Clinics for Patients Over 40
Live-birth rates fall sharply with maternal age, so the patients with the toughest odds, those in their early 40s, have the most to gain from choosing a high-performing clinic. The table below ranks the 15 U.S. fertility clinics with the highest CDC-reported live-birth rate per cycle for patients over 40, drawn from the 2022 National ART Surveillance System and limited to clinics reporting at least 100 total cycles so a single low-volume year cannot distort the list. For own-egg cycles the CDC publishes one bracket above age 40 rather than separate 41–42 and 43+ figures, so this ranking uses that band as reported; a 41-year-old and a 44-year-old sit in the same number.Source: CDC 2022 ART Report, https://www.cdc.gov/art/artdata/index.html
What the data shows
Across these 15 clinics, the over-40 live-birth rate per cycle ranges from 28.6% at the top down to 18.3%, with every one well above the national average for this age band, where most cycles do not end in a live birth. The leaders cluster geographically: 8 of the 15 are in California, and the full list spans 7 states, so strong early-40s outcomes are not confined to a single region. Because age-band figures rest on smaller cycle counts than a clinic's overall rate, treat the ordering as a shortlist of strong performers rather than a precise leaderboard, and read each clinic's full profile before drawing conclusions.

Methodology Notes
Clinics are ranked by the CDC-reported live-birth rate per cycle for the over-40 patient bracket (2022 NASS), restricted to clinics with at least 100 total reported cycles to reduce small-sample volatility. This is CDC's single above-40 band for own-egg cycles; because it is not broken down further, it cannot separate outcomes in the early forties from those later in the decade, and the underlying age mix varies by clinic. The rate is the clinic's own figure as submitted to CDC; it counts live births per cycle, not per embryo transfer or per intended retrieval, so it is not directly comparable to denominators some clinics advertise. Per-clinic figures for each of CDC's four own-egg age bands, for all 495 reporting clinics, are available via the clinic directory.
Caveats and Limitations
Age-band success rates are computed from a subset of each clinic's cycles, so they carry more statistical noise than overall rates and a single strong or weak year can shift a clinic's position. CDC figures do not adjust for patient diagnosis, prior treatment history, embryo-screening protocol, or whether a clinic accepts complex cases, and they exclude donor-egg cycles, which change outcomes substantially for this age group. A high ranking is not medical advice; consult each clinic's full profile and a reproductive endocrinologist before making decisions.
